First off, How the hell is Madara undefeated when he lost to Hashirama, and not to mention was killed by Black Zetsu or Kaguya??
Technically Aizen Sousuke hasn’t lost in the anime, he was imprisoned though still powerful af.

Honestly, if there’s an honorable mentions, 2 villains are needed who turn out to be 2 of my most hated anime villains I’ve ever seen. Ryo Asuka/Satan from Devilman series & Griffith/Femto from Berserk manga series.

Kimimaru from Naruto never really lost. He was on the verge of winning when his illness killed him. Otherwise he would’ve won
